Scary.  How do you suppose the battery got to that state in the first place?  You're not the kind to play with stupid!

Not sure - I'm guessing a hard short due to a damaged wrapper. The inside of that mod is getting coated in something non-conductive. There's no evidence of burning inside the mod but it may have been enough to get the (Chinese) cell into thermal runaway. That cell had in the high tens of cycles on it and was about a year old. No trouble till it decided to fail violently.

The reason I suspect this was that I did notice the mod getting warm so I automatically pulled the cell, saw a small hole in the wrapper at the shoulder of the cell (+ end). Then the cell started to get hotter in my hand so it was time for it to leave the house. It takes two hands to open my back door so I put the cell down on the worktop which is very fortunate as it chose that moment to blow. Go the door open, kicked the cell out the door then closed off the kitchen while I let it ventilate - the dogs were determined to go where they could get hurt of course!

Taped round the door with masking tape to keep the nasty crap in the kitchen and let it air for an hour or two.
